Registering a Death
A death must always be registered within 5 days in the district it occurred. Whilst it is usual for a relative to register a death, it can also be registered by another person present at the time of death or by an executor, solicitor or similar. The following information is required when registering a death: Medical certificate stating cause of death Any forms supplied by coroner Date and place of death Full name of deceased Previous names (if any) Date and place of birth Occupation Address Deceased’s Medical Card(if available) Details of Pensions/Allowances deceased was receiving
If the deceased was married: Date of birth of surviving partner Also needed for a woman even if widowed, separated or divorced: Maiden name Name of husband and his occupation
