A special plastic bottle, form and two stickers are supplied. You complete the form with details of any medical condition, medication etc. and put it in the bottle in your fridge. You put one sticker on the fridge door and one on the inside of your front door to alert the emergency services. Via Packs should be available at doctor’s surgeries and in some chemists.
