This directory has been compiled with older people and their carers/families who live in the Buckinghamshire area.  It has been designed as a signpost; to point out what help and information is available in the area. It is a response to a direct request from older people for co-ordinated information.

Guide to this directory

Welcome to the first edition of Agewell Information Point, a directory of services for older people and carers/families in the Buckinghamshire area.  It has been produced by Agewell, a mult-agency project group consisting of representatives from the statutory and voluntary sector working with and for older people.  The directory has been put together following requests from older people for clear and accessible information to enable them to participate in their communities and to take full advantage of services and entitlements.

Older people have been actively involved in developing this directory.
We would particularly like to thank the members of Older People’s Action Groups (OPAGs), Peoples Voices, South Buckinghamshire Carers, Chesham Asian Women’s Drop-In, Princes Risborough Day Care Centre and residents of Perry House, Burnham.

In compiling this directory we have drawn on a number of other very useful earlier publications in particular the former Elderly Persons Integrated Care Service (EPICS) Handbook, the Directory of Adult Care Services in Buckinghamshire and the Burnham Area Access Directory.

Finding your way round the directory

The information in this directory is divided into 12 sections and is arranged alphabetically within each section. You will find several organisations are referred to in more than one section but to avoid duplication of information their full contact details are not given each time. To help you find the information you require, you can search the entire contents of the directory by clicking on the search button at the top of each page.

Every effort has been made to ensure that the information contained in this directory was correct at the time of publishing but we cannot accept responsibility for any errors or omissions.

The National Service Framework for Older People

The National Service Framework (NSF) for Older People published in 2001 is a 10 year government-determined effort to deliver real improvements for older people and their families.  It set eight national standards and service models of care across health and social care for all older people whether they live at home, in residential care or being looked after in hospital.

In November 2004 the DH published Better Health in Old Age reporting on progress in implementing the NSF and in 2006 “A New Ambition for Old Age” which detailed the next steps in implementing the NSF for older people.  Next steps consists of ten programmes driven nationally.  It includes detailed plans for ensuring dignity in all care settings and at the end of life, improved services for people with strokes, falls, dementia, multiple conditions and complex needs, information technology for personalised care and for promoting health and active life, independence, well-being and choice for older people.

A Buckinghamshire wide Older People's Partnership Board with representation from LA's, NHS Trusts, the community and voluntary sector and older people champions oversees the strategic direction and commissioning of older people services.  The Board also monitors progress against the NSF and Next Steps through a series of milestones and performance measures.

A Buckinghamshire Older People Strategy 2005 – 2011 developed and led by Wycombe District Council informs the strategic direction of the broader health improvement measures to improve the health of older people in Buckinghamshire.
